NOTICE REQUIRING COMPANION ANIMAL TO BE REGISTRED


The NSW companion Animals Register Indicates that your pet has been permanently identified by a microchip but has not yet been lifetime registered.


Council requests that you register your pet within 30 days.


If youno longer own this animal , you must complete and return the attached statutory declaration form withn 30 days.


Failure to register a companion animal is an offence and may result in a penalty of $165 if this notice is not complied with.
